REFUND POLICY
The cost of the initial consult is non refundable once the visit is completed. If you choose not to continue with choosing a treatment plan, a refund will not be issued. If the appointment needs to be rescheduled it must be done more than 24 hours before the appointment time. If an account is past due, the medication will not be ordered or refilled until the account is paid up to date. Memberships can be cancelled up until 2 days prior to monthly billing on the first of the month.
REQUIREMENTS FOR APPOINTMENTS
All paperwork sent in patient portal must be completed prior to consult or appointment will be canceled and cancellation fee of $50 will be charged.
